What is my ID number?
If you are covered under one of the Trust’s medical plans, your BlueCross BlueShield UEBT ID card will show your Identification number directly below the member’s name and next to the prefix UMX.
If you are covered under one of the Trust’s dental plans, your Delta Dental of WA ID card will show your member number below the member’s name.
Where should claims be submitted?
Medical claims should be submitted to Premera Blue Cross.
Dental claims should be submitted to Delta Dental of Washington.
Vision claims should be submitted to Vision Source Plan (VSP).
Short Term Disability, Employee Life and Accidental Death and Dismemberment claims, should be submitted to United Employees Benefit Trust (UEBT) via mail, email or facsimile.

What is a preferred provider?
A preferred provider is a doctor, facility or dentist that has signed an agreement with a network.
How do I find a preferred provider?
Medical Preferred Provider:
Visit www.premera.com/sharedadmin or call 1-800-810-BLUE (2583)
Dental Preferred Provider: Visit www.deltadentalwa.com or call 1-800-544-1907
Vision Preferred Provider: Visit www.vsp.com or call 1-800-877-7195
How do I know if my doctor is a preferred provider?
You can find out if your provider is preferred by visiting the network’s website or by contacting your provider’s billing office. Visit the Links section below to access these networks.
How do I request additional ID Cards?
You can request medical ID Cards from this website (under Forms and Documents) or by calling the Trust office at (253) 474-1214.
You can request dental ID Cards from the Delta Dental of Washington website. You will need to register and login to your MySmile account at www.deltadentalwa.com
How can I obtain a certificate of creditable coverage?
A Certificate of Creditable Coverage may be requested by calling the Trust office at (253) 474-1214.
How do I obtain a copy of a Summary of Benefits and Coverage (SBC)?
The SBC for the current year is posted on the website Dashboard and included below for your convenience.
How do I obtain prescription drug information?
Please visit maxorplus.com or call 877-829-9961. MaxorPlus Member Services are available 24 hours a day / 7 days a week.
For questions regarding Mail Order Program, call 800-687-8629 Member Advocates available Monday through Friday from 7AM to 9PM, Saturday from 8AM to 6PM and Sunday from 9AM to 5PM CT (Central Time).
How many days supply of a medication can be obtained?
Medications dispensed at MaxorPlus participating retail pharmacies are limited up to a 30 day supply.
Medications obtained through mail order are limited up to a 90 day supply.
When can I obtain prescriptions that my physician has authorized refills on?
Most prescriptions are able to be refilled once 75% of the prescription has been used. For example, on a 30-day supply prescription, you may fill when 7 days are left.
